I've had the good fortune to spend a lot of time recently with an old college friend whose story provides a good lesson for creating a system of continuous improvement.
When asked about his success based on the lessons of his life, he will answer in one word.
This means careful, intentional planning; dedication to process; attention to details; and a careful analysis of results.
Maintaining discipline can be hard for a host of reasons, not the least of which is we have too much to do these days.
This is one of the reasons why software has become so prominent. is that it can automate the little things that challenge adherence to discipline problematic.
Let's take quality control, for instance.

2) Removing one activity requiring human discipline frees more capacity to remain disciplined in areas that cannot be automated.
